Horizon Family Practice
Overview
Horizon Family Practice is a small clinic with a big heart located in West Edmonton close
to Misericordia Community Hospital. With family physicians and four staff members, the clinic is affiliated with the Edmonton West PCN with a great centralized support structure comprising of dietitians, social workers, psychiatrists, counselors, and a robust lower leg assessment team run by RNs. Patients have access to virtual mental health 3 days a week. Due to the team's strong senior friendly clinic initiative in place, fall risk assessments are run on a regular basis. Each physician has 3 exam rooms and space for learners to do their charting and discussions. Each resident gets their own workspace and ample opportunity to conduct counseling sessions and detailed physicals without time constraints.
