DoM Internal Annual Review Process
This page contains information for academic and clinical faculty on how to complete their Faculty Annual Report (FAR) for the Faculty of Medicine & Dentistry and how to prepare for their annual review with the Chair of the Department of Medicine.

1. Complete FAR
Completing FAR is more than just being a bean counter — it showcases the significant impact of your work and helps us better understand and appreciate your contributions in all domains of your position description.
Deadline: September 30
- VALIDATE YOUR REPORT - to ensure all required fields have been completed, click the "Validate" button.
- Check with your Division Director before clicking "Submit", as they may prefer that you download and email your report to them.
- Ideally, your report should be SUBMITTED three business days prior to your meeting with the Chair. If your report is not complete, send your incomplete report (PDF) to eachair@ualberta.ca three business days prior your meeting.
- FAR will auto-submit your report (even if it is blank) on Sept 30 @ 11:59 p.m. MT.
- Note: the DRAFT watermark will appear on your report even after it has been submitted.
Who completes FAR?
In the DoM, only the faculty members listed below must report their activities through FAR:
- Continuing faculty (tenure-track, tenured, special continuing and FSO).
- Clinical faculty on the AMHSP (including clinical faculty with a TRAS appointment).
Complete the Faculty Annual Report (FAR)
All University of Alberta faculty members with a continuing appointment (tenure-track, tenured, special continuing and FSO appointments) are required to complete the Faculty Annual Report through the FAR portal.
Please note that failure to submit your annual report may lead to the assignment of a “0-D” merit increment. Two “0-D” merit increments in any 3-year period may lead to disciplinary action and possible dismissal from the University.
Complete your FAR by Sept 30:

- Refer to the FoMD FAR Manual for more information.
- Reporting period: July 1, 2023 to June 30, 2024.
- Please ensure you have entered your ORCID into FAR under Profile. Note: for proper import of publications, your ORCID needs to be linked to SCOPUS and Web of Science, and after you follow the instructions in the FoMD FAR Manual, it will take a few days for all of the systems to sync properly.

- VALIDATE YOUR REPORT to ensure all required fields have been completed.
- FAR will auto-submit your report (even if it is blank) on Sept 30 @ 11:59 p.m. MT.
- Note: the DRAFT watermark will appear on your report even after it has been submitted.
- If minor revisions are required after this date, you can request that your report be unlocked from within FAR.
- For Continuing Faculty, the deadline for final revisions is 5 p.m., October 25. All open reports must be submitted by this deadline.
Complete the Faculty Annual Report (FAR)
In the Department of Medicine, all Clinical faculty on the AMHSP are required to complete a Faculty Annual Report through the FAR portal.
Please note that failure to submit your annual report may lead to the assignment of a “0” merit increment (where applicable) and reconsideration of ISA renewal.

- For Clinical Faculty on the AMHSP, the deadline for final revisions is 5 p.m., November 15. All open reports must be submitted by this deadline.
2. Meet with your Division Director
- You must meet with your Division Director to discuss your 2023-24 Annual Report prior to meeting with the Department Chair.
- When completing your report, check with your Division Director before clicking "Submit" as they may prefer that you download and email your report to them.
- AMHSP members, this year you will also be completing your 2025-2026 ISA in consultation with your Division Director, prior to meeting with the Chair.
3. Annual Review Meeting with the Chair
See below for instructions on booking your annual review meeting.
Meeting with the Chair
Continuing faculty members are required to schedule an annual review meeting with the Department Chair. While there is a ZOOM option, Dr. Kassam prefers in-person meetings if possible.
This year, it is OPTIONAL for those with the academic rank of "full Professor" to schedule a meeting with the Chair. If you do NOT wish to meet with Dr. Kassam, please notify our office so that we do not follow up with you.
Note:
- You must meet with your Division Director prior to meeting with the Department Chair.
- AMHSP members, this year you will also be completing your 25-26 ISA in consultation with your Division Director, prior to meeting with the Chair.
- Ideally, your report should be SUBMITTED three business days prior to your meeting with the Chair. If your report is not complete, send your incomplete report (PDF) to eachair@ualberta.ca three business days prior your meeting.
- The Faculty Evaluation Committee (FEC) will meet to determine merit increments for continuing faculty from January 20–24, 2025.

Optional Meeting with the Chair
AFTER you have met with your Division Director, you have the OPTION to meet with Dr. Kassam.
- Appointments (15 minutes in length) are available on October 28, 29, 31 and November 12 and 13, 2024.
- Dr. Kassam prefers in-person meetings, however Zoom is also an option.
- Use this link to book your meeting by October 10.

For Clinical Faculty on the AMHSP, decisions around merit increments (where applicable) will be made by the DoM Clinical Faculty Evaluation Committee (CFEC) in spring 2025.
