GPAP (Geriatric PD assessment)

Geriatric PD assessment is a new program in the PMDP, offering Comprehensive Geriatric Assessment and Management of motor, non-motor symptoms of Parkinson disease, multi-morbidities, frailty and medication assessment for older adults with Parkinson Disease/parkinsonism.

Patients will have access to our team members, including telephone support for patients and caregivers between appointments by our nurses, to optimize well-being and function, and to prevent unplanned hospitalization.

The program aims to include a Geriatric Pharmacist in the near future, and to expand the delivery of optimal care for older adults with Parkinson disease through the province through education and support for Primary Care Networks.
