Heart Failure Disease Care Path Integration-
Evidence-based Guideline Recommended Best Care
Heart Failure (HF) is a complex syndrome and a major public health problem. Recent (2021) updates in the Canadian Cardiovascular Society guidelines for the management of HF define a new standard of pharmacologic care for HF with reduced Ejection Fraction (HFrEF) that incorporates 4 key therapeutic drug classes as standard therapy for most patients. These foundational Guideline Directed Medical Therapies (GDMT) will be reviewed in addition to secondary therapies to improve outcomes in patients with HFrEF. Furthermore, recent evidence for Sodium-Glucose Cotransporter-2 Inhibitors (SGLT2i) for HF patients with preserved ejection fraction (HFpEF), regardless of the presence or absence of diabetes, will be presented. HF diagnosis, pathophysiology, and treatment options will also be reviewed.
This free accredited eLearning course provides an overview of Care Paths in Connect Care and details the HF-specific Care Path, identifying benefits and clinical decision supports.

- RCPSC - This activity is an Accredited Self-Assessment Program (Section 3) as defined by the Maintenance of Certification Program of the Royal College of Physicians and Surgeons of Canada, and approved by the University of Calgary Office of Continuing Medical Education and Professional Development. This activity has been approved for a maximum of 1.5 hour (credits are automatically calculated)

- Practice highlights, summary, and data dissemination with practice reflection questions will be emailed for your self-review.

Caregiver-Centered Care Education for Healthcare Providers
About the course:
Family caregivers' care work is crucial to sustain the health and social care system. There is a need to identify, engage, and support family caregivers through a person-centered approach that:
- Respects and meaningfully integrates family caregivers as equal partners in patient-care; and
- Assists family caregivers to maintain their care and own wellbeing.
Through co-design with over 100 multi-disciplinary professionals and family caregivers, Caregiver-Centered Care education builds health and social care providers' competencies to better support and partner with family caregivers.
This free, self-paced online education is evidence-based, and provides a deeper understanding of the essential building blocks to achieving Caregiver Centered Care outlined by the Caregiver-Centered Care Competency Framework ©.
A Caregiver -Centered Care approach builds meaningful and positive care outcomes for everyone involved in the care journey.
- Recognize the value, impacts, roles, and diversity of family caregivers.
- Communicate effectively with family caregivers.
- Partner with family caregivers.
- Foster family caregivers' resilience on an ongoing basis.
- Navigate the health and social systems with family caregivers.
- Enhance the culture & context of care.
- Foundational Education (Level 1): expected to take the learner 1-1.5 hours to complete all 6 modules
- COVID-19 Education: will take the learner approximately 40-45 minutes to complete all 3 modules.
- Advanced Education (Level 2): expected to take the learner 1-1.5 hours to complete each of the 6 modules.
