Evaluation Forms
An End of Term Employer Evaluation is required for each 4-month work term in order for a student to obtain credit. More importantly, feedback is important to the student’s development and future success in engineering.
The Process
Supervisors will receive a unique link via email to complete the evaluation form in campusBRIDGE. Emails will be sent no later than three weeks prior to the end of each 4-month work term block. Important: You do not require a campusBRIDGE account in order to complete the form, but you will need to use Chrome, Firefox, or Safari as your browser. If the evaluation form is left open for 30 minutes it will time-out and any information entered will be lost. Be sure to intermittently scroll to the bottom and click the ‘Save as Draft’ button to ensure no loss of data.
Issues with the campusBRIDGE link?
If your evaluator experiences any issues with using the link to the evaluation form in campusBRIDGE, please have them complete this fillable PDF form. Once the form is completed, you and your supervisor will have a discussion about it. After this occurs remind them to:
- Save the completed evaluation form using the following naming convention: ID#_EREV (e.g., 1234567_EREV).
- Email the signed PDF to the co-op team member that conducted the onsite evaluation, copying you, by no later than the last day of the current 4-month work term.
