It is the student's responsibility to print the form. Please follow the instructions on the form and complete it to the best of your ability. You may need to consult with the instructor of the course for which you require the form. If you are unsure or need help completing your form, please contact the Undergraduate Administrator or Undergraduate Advisor.

Completed, original forms should be either mailed or hand-delivered to the Main Office of the Department of Women's and Gender Studies.
