Enhanced HR Forms for Academics and MAPS Employees

As part of the ongoing Continuous Administrative Service Improvement Program (CASIP) initiative to review and improve HR forms and checklists, Shared Services has made changes to the following HR forms. All forms have been updated to have a consistent layout that aligns with form standards and U of A branding in addition to the specific form updates noted in the summary of changes.

  • Notice of Retirement_Temp Academics and MAPS - updated & renamed 
  • Notice of Retirement_Continuing MAPS - updated & renamed 
  • Notice of Retirement_Continuing Academic - archived 
  • Supplementary Retirement Plan Beneficiary Designation - updated 
  • Supplementary Retirement Plan Investment Changes - updated 
  • HR Development Fund Application_Support - archived
