Updated Legal Name Change Process for Faculty and Staff

A new Faculty & Staff Legal Name Change form is available for current or former university employees who wish to change their legal name.
At times, the university may be required to use a faculty or staff member’s legal name in the course of its communications, particularly in cases related to employment, payroll, etc., or where a legal name is required by a third-party organization. Faculty or staff members who wish to change their legal name with the University of Alberta can complete a Faculty & Staff Legal Name Change Request form and submit through the Staff Service Centre.
Please note the following:
- This form is not required to update a preferred or primary-affirmed name. These updates can be completed by updating the information in Bear Tracks.
- Students who are seeking to change their legal name should refer to the Office of the Registrar’s Changes to Personal Information page.
