Intensive Community Transitions Program

Intensive Community Transitions Program supports and advocate for clients experiencing addiction and mental health issues who are within the Inner City and/or leaving remand and/or correctional centers. The program represents a holistic and collaborative approach; it serves as a bridge from our inner city or custody to the appropriate services within the community.   Currently delivers specialized addiction and/or mental health consultation and case management (as needed) services to multiple inner-city agencies listed below. Clients who are homeless and experiencing significant addiction and/or mental health concerns are referred to the team via an identified agency staff member. ICT receives referrals and provides in-reach into the following agencies:

  • Women’s Emergency Acc Shelter (WEAC)
  • George Spady Society
  • Herb Jamieson
  • Hope Mission
  • Boyle McCauley Health Centre
  • Boyle Street Community Services
  • Bissell Centre
  • Other Inner-City Agency Partners
  • RAH Emerge Mental Health
  • Remand
  • Fort Edmonton Correctional Center

The ICT program has support of a multidisciplinary team that consists of:

  • Mental Health Therapists
  • Occupational Therapists
  • Social Workers
  • Addiction Counsellor
  • Psychiatry Support Workers
  • Independent Living Support Workers
  • Peer Support Workers
  • Admin staff
  • Psychiatrists

The ICT Program offers medical students and psychiatry residents the unique opportunity to learn about multidisciplinary team working and participate in client centred care for patients with severe and persistent mental illness when they live-in inner-city settings or are homeless and/or have interactions with the criminal justice system.
